Your initial appointment in our San Jose or Fremont Endodontic office will consist of a consultation explaining your diagnosis and treatment options. During consultation visit, we will review & discuss your chief complaint, detailed tooth pain history, medical and past dental history & dental phobia (if any). In addition, a radiographic exam & pulp vitality tests will be performed to analyze the pulpal & periradicular status of the tooth in question. We require most recent & also different angled x-rays to obtain all the necessary details. Hence, even though you may bring your own set of x-rays, it is sometimes necessary to take additional 2 dimensional & 3 dimensional CBCT images in our office to obtain all the necessary information to help us achieve the right diagnosis.
Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance bring the necessary insurance card & information. This will save time and allow us to help you process any claims.
IMPORTANT:All patients under the age of 18 years of age must be accompanied by a parent or legal guardian.
